and telework. Job duties require at least two (2) days a week
onsite at assigned health center site/s. Job duties also require
in-person meetings and collaboration with other staff at PCC. Note:
PCC employees must reside in the following five states or District
of Columbia: MD. VA, WVA, DE. PA, DC. Relocation expenses are not
reimbursed Additional Incentives: PCC will cover expenses related
to obtaining a Maryland clinical license (up to $500) for eligible
credentials (LMSW, LCSW-C, LGPC, LCPC, Psychologist, RN). Covered
costs include exam fees, background checks, and related charges.
This benefit applies to employees who do not already hold an active
license, with the requirement that the license be obtained within
120 days of hire. PCC also covers the cost of license renewal for
employees. The Bilingual Behavioral Health Care Manager (BBHCM)
will be responsible for offering behavioral health services to
underserved patients living in Montgomery County. This position is
as a team member with the Montgomery Cares Behavioral Health
Program (MCBHP). This position is best suited for those who are
passionate about health equity and enjoy collaborating with other
providers to offer behavioral health services in a setting that is
integrated with primary care. As a team member within the
Collaborative Care Model, you would support primary care providers
by identifying, treating and tracking patients with common
behavioral health problems such as depression and anxiety. In
addition to being part of a multidisciplinary medical team, you
would also be part of a broader behavioral health team supporting
patients across Montgomery County. Our Montgomery Cares Behavioral
Health Program (MCBHP) model makes behavioral health care more
accessible for those who need it most. You will be part of a team
that is working to advance health equity for vulnerable patients.
This position is a great fit for someone who wants to maximize time
on direct service, as the position does not require billing,
coordinating with insurance companies or Medicaid/Medicare, or
preauthorization. Additionally, while we focus on meeting patient
need, there is no quota or compensation tied to productivity.
Essential Duties 1. Provide clinical services, utilizing the
Collaborative Care Model of integrated behavioral healthcare.
Performs and documents biopsychosocial evaluations, seeks
psychiatric team consultation, and develops and implements
treatment plans with patients Reassesses and monitors patients’
response to treatment on a regular basis using program instruments
to track patient response to medication and/or other treatment
Provides psychoeducation, behavioral activation and group and
individual therapy to patients on an as needed basis as part of
their treatment plan (mix of telehealth and in-person services)
Identifies appropriate resources, and coordinates referral process
to community resources or behavioral health resources to meet their
treatment goals Provides on-site and virtual crisis management and
referrals, when necessary, in consultation with primary care
providers, Supervisory Behavioral Health Care Manager, Director and
consulting psychiatrist. 2. Serves as representative of behavioral
health at assigned site/s and works with site partners to ensure
optimal operations of integrated behavioral service provision.
Ensures that the MCBHP functions effectively and efficiently at the
assigned primary care health center site/s. Ensures that MCBHP
Policies and Procedures are implemented and followed Serves as the
main liaison between the MCBHP staff and other key people Oversees
the productivity of the MCBHP at the host site, and when
appropriate take measures to ensure that access to services is
available to patients and that target goals for serving patients
are met Serves as the primary liaison with the psychiatric
consultants, patients, and the primary care provider regarding
patient care Assists assigned sites with workflow and policies on
crisis intervention and other behavioral health topics Additional
Responsibilities Participates in staff meetings of the MCBHP,
partner health center sites, and PCC • Attends appropriate
trainings and meetings to improve skills in appropriate areas • May
supervise interns or provide clinical supervision to graduate level
providers as appropriate. Provides technical assistance and
education to assigned health center staff on behavioral health
topics Contributes to program publications and PCC committees and
workgroups • Other duties as assigned including providing coverage
for other staff if needed Skills and Abilities Ability to develop
and maintain trusted relationships with patients, providers, and
other members of the multidisciplinary team Excellent
organizational and time management skills and ability to manage a
caseload of patients with varying complexity of needs Excellent
oral and written communication skills, including ability to
document in an electronic health record Self-motivated with the
ability to work independently Education and Experience LMSW, LGPC,
LCSW-C, LCPC, Licensed Psychologist or BSN Required, including
active license OR Maryland license obtained within 120 days of hire
At least one year of professional experience (may include graduate
school experience) working in mental health or social services with
underserved populations Required Lived or working experience, and
sensitivity to, the needs of immigrant and other vulnerable
populations Bilingual in English and Spanish Required Full
independent clinical licensure (i.e. LCSW-C or LCPC) Proficiency in
one or more additional languages besides English and Spanish (eg.
Amharic, French, or Portuguese, desirable) Approved by the Board of
Social Workers or the Board of Counselors and Therapists as a
clinical supervisor ADA Requirements This job operates in a
